Please arrive 15 minutes early to fill out paperwork.

Bring your insurance card on your first visit and make sure to bring a new card if you have been issued one by your carrier.

If you were given an order, please bring this with you.

Your first visit will include an evaluation with one of our Physical Therapists lasting about 1 hour.

​

Each visit after this will usually be 30-45 minutes but could last about 1 hour if extra attention is needed and scheduling allows.
